Payment Options
- Payment is expected at time of service using MasterCard, Visa, Cash, Check or Insurance.

- If you have insurance, co-pays and deductibles are due at time of service.
- If you have insurance and do not pay your portion promptly, we will not file your insurance for you.
- Any accounts past 90 days will be sent to a collection agency which may adversely affect your credit score.

Broken Appointment Policy
- Please give our office a 24 hours notice if you cannot keep your dental appointment.
- After three (3) broken appointments with no 24 hours notice given, you will be dismissed from the practice.
- Lateness (10 minutes or more) may result in rescheduling your dental appointment. Chronic lateness may result in dismissal from practice.
