Payment Options
 1) Payment is expected at time of service using MasterCard, Visa, Cash, Check or Insurance.
2) If you have insurance, co-pays and deductibles are due at time of service.
3) If you have insurance and do not pay your portion promptly, we will not file your insurance for you.
4) Any accounts past 90 days will be sent to a collection agecy which may adversely affect your credit score.

Broken Appointment Policy
1) Please give our office a 24 hours notice if you cannot keep your dental appointment.
2) After three (3) broken appointments with no 24 hours notive given, you will be dismissed from the practice.
3) Lateness (10 minutes or more) may result in rescheduling your dental appointment. Chronic lateness may result in dismissal from practice.
